Pizzi Social Environment Interview (PSEI)
The PSEI was developed when I worked with a woman with terminal cancer and her daughter was unable to organize her own life, time and occupations. The client took me aside one day and asked that I help her daughter! Thus the PSEI was developed to help the caregiver understand the importance of examining the depth of one’s own wellness in the face of a loved one’s illness.

The PSEI, along with the PHWA, can be used for ANY caregiver in need of rebalancing his/her life and re-examining health and well-being for him/herself.

Pizzi Hospice Assessment of Occupational Participation (PHAOP)
This assessment was developed from my many years working with people with life threatening illness, and based on principles of occupational therapy, occupational science, social rehabilitation, wellness, and health promotion. I discovered that there was no ONE assessment that covered the many areas of life participation for which therapists need data to develop a holistic and life enhancing program. This assessment explores many areas of life, and can be administered in one or several sessions. The questions can also be asked of caregivers for a more well-rounded idea of the person in order to best meet their needs and help them to live a life of quality and dignity.

Pizzi Occupational Loss Assessment (POLA)
In the early 90’s, after many years of work with the terminally and chronically ill, I recognized the lack of information, and the importance of knowledge in, people’s perceptions of loss. The impact of loss on life participation is profound, yet we do not formally assess it or develop interventions around it as it impacts on life. These losses may include the loss of function, the loss of a person, or the perceived loss of what a relationship could have been. This assessment is unique as it explores not only the loss, but is a self rating tool and examines the impact of the loss, from the clients perspective, on life participation.

Pizzi Holistic Wellness Assessment (PHWA)
Being able to do daily tasks that hold meaning for you creates a sense of balance in your life and a healthy way of living -- esentially, it renews your total way of being.

Wellness is having a life in balance. This includes doing things in your life on a daily basis that you enjoy doing or feel good about accomplishing by yourself. Most people enjoy a sense of accomplishment, or wellness, by carrying out tasks independently and feeling positively about having done them. (Depending on your physical condition, these tasks may be as complex as managing the stress of an eight-hour day at work.)

The following pages will introduce you to that sense of balance. You will explore The Pizzi Holistic Wellness Assessment (PWHA). The goal of the PWHA is to help you identify your current level of wellness or balance. By exploring each of the "branches" of the spiral, you will begin to see how each factor (your spiritual, sexual, physical, family, social, occupational, intellectual, and psychosocial levels) contributes to your wellness.

The worksheets that follow the spiral will identify challenges and developmental areas that will help you bring those factors you wish to focus on into "alignment". Alignment, balance, and wellness help you acheive a sense of well being, which in turn leads to a full and happy life. It is important to experience a sense of renewal and accomplishment on a daily basis.

An additional spiral has been provided for you along with the related worksheets. Use it to gain another person's opinion of your wellness by asking the person closest to fill out the PWHA. The person you select should keep you in mind as they complete the exercise. It is important for them to be honest. This person plays an important role in your wellness, so be sure to include them. Their feedback can be very helpful.

Wellness, not illness, must be the key focus of daily living in order to promote healthy activity and a healthy life.
